Note what this does not say. It does not say the hospital fails Spanish-speaking patients. The staff are local, the hospital’s non-discrimination notice commits it to qualified interpreters and to written information in other languages, and care at the bedside is very plainly delivered bilingually. What it says is narrower and more useful: the written layer is English-first while the spoken layer is bilingual. The burden of translation therefore falls on staff, live, one patient at a time, repeatedly — and it falls hardest exactly where written material matters most and staff time is scarcest: discharge instructions, consent, and the bill. That work does not appear in any staffing model, it is not compensated, and it falls on whoever is most fluent.

So anything a patient is expected to read after they go home has to work on a phone, over cellular — and it also has to exist on paper. A portal-only discharge experience excludes several thousand households here.
